Player Release of Liability and Assumption of Risk

Edge Americas Sports Inc. – Juventus Academy Los Angeles – LA Youth – LA10 FC – DNA Prep Academy – CAMPO

 

1. Assumption of Risk

By participating in soccer-related activities organized by Edge Americas Sports Inc. and its affiliates, including training sessions, matches, clinics, and events ("Soccer Activities"), you acknowledge and accept that such activities involve inherent risks of physical injury, illness, disability, or death.

You agree that the Player and their family derive meaningful benefit from participating, and by proceeding, you knowingly and voluntarily accept all risks—both known and unknown, anticipated and unanticipated—even if arising from the negligence of Edge Americas Sports Inc. or any of its affiliated entities.

2. Release of Liability

You, on behalf of yourself, the Player, and your respective heirs and legal representatives, hereby release and hold harmless:

Edge Americas Sports Inc., Juventus Academy Los Angeles, LA10 FC, DNA Prep Academy, CAMPO, and all associated directors, officers, employees, volunteers, agents, sponsors, advertisers, venue owners, and affiliates (“Releasees”)

…from any and all claims or liabilities resulting from injury, illness, death, property damage, or other losses arising from the Player’s participation in Soccer Activities, whether based on negligence, contract, tort, or other legal theory—even if caused by the actions or inactions of the Releasees.

3. Health & Fitness Acknowledgment

You confirm that the Player is physically fit and capable of safely participating in Soccer Activities. If any health concerns arise, you agree to immediately remove the Player and notify Edge Americas personnel. You understand that Edge Americas does not control or guarantee the safety of any facility used.

4. Media Release

By participating, you grant the Releasees the royalty-free, worldwide, and perpetual right to use the Player’s and your name, image, likeness, voice, and appearance in all media formats—including web, social media, print, and promotional campaigns—without compensation.

You also consent to the collection and distribution of contact information, including email, for promotional and operational purposes.

5. Insurance & Medical Responsibility

You acknowledge that the Releasees do not assume responsibility for medical costs or treatment. You certify that the Player is covered by valid health insurance that includes coverage for injury or illness occurring during Soccer Activities in California.

If not insured, you accept full responsibility for all medical costs incurred and agree to arrange immediate payment with providers. You also authorize emergency medical treatment as deemed necessary by healthcare professionals, including hospitalization, surgery, anesthesia, and dental care.

6. Arbitration Agreement

Any dispute related to this agreement or the Player’s participation in Soccer Activities shall be resolved through binding arbitration administered by JAMS in Los Angeles, California, in accordance with JAMS rules.

By accepting this agreement, you waive your right to bring legal action in court, including the right to a jury trial. The prevailing party in arbitration shall be entitled to reasonable attorney’s fees and costs.

7. Communicable Disease Waiver (Including COVID-19)

Participation in Soccer Activities may expose the Player to infectious diseases, including but not limited to COVID-19, MRSA, and influenza.

By proceeding, you knowingly and voluntarily assume all risks related to such exposure, even if caused by the negligence of the Releasees. You agree to follow all safety protocols and immediately report any health concerns. You further agree to release, defend, and indemnify the Releasees from any claims related to exposure or illness resulting from participation.

 

Digital Consent

By checking the acknowledgment box during registration or form submission, you confirm that:

• You are the parent, legal guardian, or authorized representative of the Player.

• You have read and understand this agreement in its entirety.

• You agree to all terms and conditions stated above.

• You acknowledge that this digital acceptance carries the same legal effect as a physical signature.
